All students entering USD 253 for the first time should submit proof of a health physical by the first day of school. All students are required to have a complete immunization record in their school health file. Please make sure we have complete records for your child's medical file.
Immunizations Needed for Kindergarten:
5 DTP
4 Polio
2 MMR
3 HBV, properly spaced doses
Varicella Vaccine or history of Chicken Pox illness
