Service Provided: Patricia Fernandez is a Regional Case Manager for the Kansas Statewide Farmworker Health Program   She is responsible for the eastern region of Kansas helping low income, uninsured farmworkers and their families access primary care services.  She is bilingual in Spanish and English.

Service Provided:  A program of KDHE, primarily for children under 21 with physical disabilities or chronic disease.  Diagnostic evaluations are available for any child from birth to 21 years regardless of family income.  (Prior approval is required by calling SHS).  SHS may pay for all or part of the cost of treatment services.  Coverage is based on family income and the anticipated cost of the health services.  Not all services have income eligibility or even age requirements.  Persons in need of services are considered individually when determining both medical and financial eligibility.  Treatment services include medical specialists, outpatient care, hospitalization, surgery, durable medical equipment, and reimbursement for interpreter services and transportation to medical specialty care.  A limited amount of speech, physical and occupational therapy is provided for eligible conditions.  (Prior approval required for all treatments).  Some conditions that are eligible for treatment include: burns, cleft palate/cleft lip; craniofacial anomalies; gastrointestinal or genitor-urinary conditions requiring surgery; hearing loss; heart disease (acquired or congenital); juvenile rheumatoid arthritis; major orthopedic problems; seizures; spina bifida; and also including a list of genetic and metabolic conditions and vision disorders.
